Practice Area

If you live within our practice area, you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

To begin, please enter your postcode below to check whether you live within the surgery's specified inner boundary area. 

Unfortunately, we are not able to take on any new patients, currently, that live outside of the practice area.

Please note that acceptance on to the list is as at the discretion of the Doctor and there are a number of reasons why you may not be able to register with your chosen GP, for example, you may live too far away or outside of the practice area. If this is the case simply choose another GP in your local area.

New Patient Registration

Please complete the online registration form or inquire at reception. The practice will then process your registration, which normally takes between 5-7 working days. We are not able to register patients without this form. If you prefer, you can collect a paper version of our Registration Form from Reception. Please note that the form requires two forms of identification and in order to process the form we require you to attend the surgery in person, to verify your ID.

The ID required, needs to be one form of photographic identification (e.g. Passport/Driving License) and one form of proof of address (e.g. Bank statement/Utility Bill).

To complete the registration please click on the link below for the form and answer all the questions, which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

Named GP

All Patients registered with the surgery will have been allocated a named accountable GP. A patient’s named GP must take lead responsibility for ensuring any necessary and appropriate services are coordinated and delivered to them.

The Named GP does not assume 24hr responsibility for the patient nor take on the work of other doctors or health professionals. Patients do not need to see their named GP at the practice and may be offered appointments with other clinicians.

If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know or change your named GP, please submit a reception enquiry.

If you wish to know which GP has been allocated to you, please ask reception at your next visit.

 

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
